Written for whoever inherits it, including the parts that are held together loosely and why.

How it runs, in order.

One

A call, about half an hour

You describe the work. I say whether I am the right person for it. If I am not, I say so on that call.

Two

The scope comes back in writing

Shape, boundaries, and the date. Nothing starts until you have read it and agreed with it.

Three

It gets built where you can see it

You have access while it is being built. There is no reveal at the end, and no week where you are guessing.

Four

Handover

Running code, the source, the access, and the description. Then a window to come back with what broke.
